Moto Fan Pages contact form

Hello. I currently use the MotoFanPages app to create a tab in my fan page. The tab was created successfully. But, i want to create a contact form so that my fans fill in their name and their e-mail. But, i want these information to be saved in a database, or a mail list. How can i connect the facebook tab with my database or mail list? Are there any related settings in the MotoFanPage app?
